Discuss it with your doctor
Tell your doctor during your consultation that you need a medical certificate and explain the reason you require one.
Your doctor completes it
If your doctor considers a certificate appropriate, they will complete it as part of your consultation.
A Few Important Things to Know
Medical certificates are medical documents, so there are a few requirements doctors need to follow.
Ask during your consultation
If you need a medical certificate, please let your doctor know during your consultation so it can be discussed and completed at the appropriate time.
Certificates cannot be backdated
Our doctors cannot provide backdated medical certificates. If you think you will need a certificate, please raise this with your doctor during your consultation.
Certificates are not automatic
A medical certificate can only be issued when your doctor considers it appropriate based on your medical circumstances.
Forgot to Ask for a Certificate?
If you have already attended your consultation and forgot to request a medical certificate, please contact our reception team as soon as possible.
