Your Feedback Matters: Patient Survey

At Southern Doctors Clinic, your feedback matters to us.

As part of our upcoming practice accreditation later this year, we are currently inviting patients to participate in a brief Patient Feedback Survey conducted through CFEP (Client-Focused Evaluation Program).

The survey allows you to share your experience with our practice and helps us understand what we’re doing well, as well as where we can continue to improve. Patient feedback is an important component of the accreditation process and plays a key role in ensuring we continue to provide high-quality, patient-centred care.

Patients may receive an email invitation after their appointment to complete the survey online. Alternatively, a paper copy is available at reception if you prefer to complete the survey in person.

The survey is completely anonymous, and responses are provided to us in a summarised report only. Individual responses are not identified.

If you are invited to participate, we would greatly appreciate it if you could take a few minutes to complete the survey. Your input helps shape the future of our practice and the care we provide to our community.
