Important Changes to Care Plans from 1 July 2025

We want to inform our patients about significant changes to how care plans are managed at our practice as of 1 July 2025. These updates are part of a national effort to simplify and improve management of chronic health conditions for patients.

What’s Changing?

  • A new, simpler care plan: From 1 July 2025, the current system using separate GP Management Plans (GPMPs), Team Care Arrangements (TCAs), and reviews will be replaced with one easier plan – the GP Chronic Condition Management Plan (GPCCMP).

  • New referral process: Allied health referrals (for physiotherapy, dietetics, etc.) will now be given as a referral letter, not on the old EPC form.

  • If you're registered with MyMedicare: You’ll need to have your care plan and reviews done at the same clinic where you're registered.

  • No change to who qualifies: You’re still eligible for a care plan if you have a chronic health condition (lasting or expected to last over 6 months) or a terminal illness.

  • No change to allied health limits: You can still get up to five Medicare-covered allied health visits per year.

How This Affects You

  • If you already have a care plan: You can continue using your current care plan until 30 June 2027. After that, you’ll need to switch to the new GPCCMP to keep accessing services.

  • Better team-based care: This new system is designed to help your GP and other providers work more closely together – so your care is more connected and easier to manage.

  • Reviews still needed: To keep using allied health services under your care plan, you’ll need to have your GPCCMP set up or reviewed within the last 18 months.

What Should You Do?

  • Check your MyMedicare status: If you haven’t registered yet, consider signing up for better continuity of care.

  • Talk to your doctor: At your next visit, ask if your care plan needs to be updated.

  • Keep up with review appointments: This keeps your plan active and ensures you keep getting support services.

Why Are These Changes Happening?

The goal for these updates is to:

  • Simplify and modernise care planning.

  • Promote regular review of chronic condition management.

  • Make care more continuous and coordinated for all patients.
